Include the KELN page address, the information you believe is incorrect, and a link to an official source if available. We check corrections against public CMS records and clarify or update our presentation where appropriate.

Do not send medical records, Social Security numbers, payment details, or other sensitive personal information. KELN cannot provide medical or legal advice, arrange care, or answer questions on behalf of a facility.
